Inclusion criteria

Inclusion criteria: 1. Age >= 60 years; 2. Diagnosis of stroke, confirmed according to the diagnostic criteria for cerebral infarction from the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2018) and for cerebral hemorrhage from the Chinese Guidelines for the Diagnosis and Treatment of Cerebral Hemorrhage (2019); 3. Being in the stable phase of the disease; 4. A score of >= 5 on the Chinese version of the Tilburg Frailty Indicator (TFI); 5. Ability to understand and respond to the investigator's questions effectively, with no communication barriers; 6. Provision of informed consent and voluntary participation in the study.

Exclusion criteria

Exclusion criteria: 1. Patients with severe mental disorders; 2. Patients suffering from other irreversible severe diseases.
