intervention group:1. Patients with dementia 1.1 Drug Review: Three fixed-procedure drug reviews (baseline T0 , T1 after 3 months and T2 after 6 months) will be conducted by the general practitioner

Inclusion criteria

Inclusion criteria: 1. Age >=60 years old; 2. Diagnosed as dementia by a specialist doctor in accordance with the criteria of the 11th Revision of the International Classification of Diseases (ICD-11); 3. There is a stable caregiver, that is: a relative caregiver has continuously provided care for >=3 months, or a paid caregiver has continuously provided care for >=1 year; 4. The caregivers of the patients are willing to participate in this study and have the ability to use the common functions of the smart phone under guidance; 5. Have resided in Beijing for more than six months.

Exclusion criteria

Exclusion criteria: Hearing or vision impairment that cannot be improved by assistive devices such as hearing AIDS, magnifying glasses, reading glasses, and near (far) glasses; 2. Suffering from severe mental disorders, serious cardiovascular and cerebrovascular diseases that have not been effectively controlled, or malignant tumors, with an expected lifespan of less than one year as determined by researchers; 3. Due to fractures, trauma, acute exacerbations of chronic diseases, etc., hospitalization is required during the research period; 4. In a severely disabled state, unable to cooperate in completing offline evaluations and home-based cognitive stimulation interventions; 5. Long-term residence in the care environment of institutions such as nursing homes and care centers.
