Interventions

Intervention group:The intervention group received the developed nursing care protocol for calluses in elderly diabetic patients: 1. Standardized callus debridement (using an electric hard skin remove
2. Customization of personalized pressure-redistributing insoles and guidance on their use
3. Foot skin care education (moisturizing, inspection, proper footwear and hosiery selection)
4. Integration of traditional Chinese medicinal foot baths or topical ointments as needed.
Control group:The control group received the existing conventional care: 1. Routine scalpel debridement
2. Conventional health education (verbal instructions)
3. Referral to dermatology or podiatry clinics for management in cases of callus thickening or discomfort
4. No standardized debridement, no customized insoles, and no structured education program.

Inclusion criteria

Inclusion criteria: 1. Age >= 65 years; 2. Meeting the 1999 WHO diagnostic criteria for diabetes; 3. Patients presenting with calluses; 4. Voluntary participation in this study with provision of written informed consent.

Exclusion criteria

Exclusion criteria: 1. Amputation of the affected limb; 2. Patients presenting with severe acute or chronic diabetic complications such as ketosis, acute infection, severe retinopathy, severe cardiopulmonary insufficiency, or nephropathy; 3. Patients with existing diabetic foot; 4. Subjects who were unable to cooperate due to mental disorders, cognitive impairments, or other conditions.
