Inclusion criteria
Inclusion criteria: 1. Age over 40, gender not limited; 2. Source: Residents from the communities under the jurisdiction of the community health service center who are willing to participate in this project and can receive basic diagnosis and treatment for hypertension, dyslipidemia and diabetes at designated medical institutions during the project implementation period. 3. Those who belong to the medium or high-risk groups based on the Chinese-PAR model of the Chinese cardiovascular risk assessment tool, or have been diagnosed with atherosclerotic cardiovascular disease Patients with disease,ASCVD (including ischemic stroke, coronary atherosclerotic heart disease, and peripheral vascular disease). 4. Those who have a clear diagnosis of at least one of the following: hypertension, dyslipidemia, or diabetes, and at least one of them fails to meet the standard. The specific diagnostic criteria are as follows: Hypertension: Any one of the following three conditions can be diagnosed as hypertension: A. Without the use of antihypertensive drugs, if the systolic blood pressure is 140 mmHg and/or the diastolic blood pressure is 90 mmHg or above in three measurements on different days, it can be diagnosed as hypertension. B. The patient has a previous history of hypertension; C. Currently taking antihypertensive drugs. Type 2 diabetes: If there are symptoms of metabolic disorders (including polydipsia, polyuria, polyphagia and unexplained weight loss), fasting blood glucose >=7.0mmol/L, or a clear history of diabetes, or if one is currently taking hypoglycemic drugs. Dyslipidemia: LDL-C in the general population is >=4.1 mmol/L, or the LDL-C control target is not reached according to the ASCVD risk stratification. Blood pressure not up to standard: systolic blood pressure >=140mmHg and/or diastolic blood pressure >=90mmHg; LDL-C not meeting the standard: The target value of LDL-C is not reached according to the ASCVD risk stratification. Blood glucose not reaching the standard: FPG>=7.0mmol/L. 5. I or with the help of my family members can use regular applications such as smart phone apps and information reception and reading.
Exclusion criteria
Exclusion criteria: 1.Patients with cardiovascular diseases (such as myocardial infarction, stroke, etc.) that have occurred within three months; 2. Patients with malignant tumors or other serious systemic diseases; 3. Those who are unable to take care of themselves; 4. Inability to cooperate with health management and follow-up for various reasons.
