Inclusion criteria

Inclusion criteria: 1. Age >= 60 years; 2. Residing in a nursing home for >= 3 months; 3. Having at least one chronic disease confirmed by a medical institution; 4. Able to understand and complete the questionnaire; 5. Informed consent has been signed.

Exclusion criteria

Exclusion criteria: 1.Have severe cognitive impairment or mental illness; 2.In a state of acute severe illness or difficult to cooperate with the investigation.
