Experimental Group:The study intervention was managed by the combined care model of "patients' family, community health service center and general hospital" based on the Internet platform (wechat). Th
Control Group:Routine Management: Intensive blood sugar control during hospitalization, along with lifestyle intervention guidance, followed by primarily telephone follow-ups post-discharge.

Inclusion criteria

Inclusion criteria: 1. Age 18-65 years old Type 2 diabetes mellitus was diagnosed 3. History of diabetes =25 kg/m^2 (or waist circumference >90cm for men and >85cm for women) 5. Fasting C-peptide >=1.1µg/L, 2h postprandial C-peptide >=2.5µg/L 6. Suboptimal glycemic control in the past 3 months (HbA1c > 7%) 7. I can adhere to and follow the education and management contents pushed by the research team through the wechat platform 8. Voluntarily sign informed consent

Exclusion criteria

Exclusion criteria: 1. Pregnant and lactating women 2. Plan to migrate and cannot be followed continuously in the study center 3. Patients with definite systemic diseases, including (but not limited to) history of ASCVD (coronary heart disease, ischemic stroke, peripheral artery disease) events or heart failure or pulmonary embolism; Blood system diseases such as anemia; Abnormal renal function: serum creatinine exceeded the upper limit of normal value, or urine protein ++ (after reexamination); Significant abnormal liver function: transaminase and/or total bilirubin >= 3 times the upper limit of normal; Rheumatic diseases: patients receiving glucocorticoid and immunosuppressive therapy; Respiratory disease: patients with severe lung disease and currently treated with oral corticosteroids; Abnormal endocrine function: such as adenopituitary insufficiency; Hiv-positive (self-reported); Cancer: cancer patients treated within the past 5 years; Organ transplant patients; Drug, alcohol, substance abuse, mental disorders and other conditions that the researcher considered inappropriate to participate in this study
