Control Group:Do not wear equipment and maintain traditional health community care.
intervention group:Wear a smart ring (wear it all the time) and CGM (wear it for 14 days during the baseline period, 14 days during the intervention period, and 14 days

Inclusion criteria
Inclusion criteria: 1. Patients aged >= 60 years and 50% (ICD-10: I20.801, I25.103); or patients diagnosed with ischemic stroke (ICD-10: I69.300, I63.900); 4. Whenever possible, priority for enrollment will be given to patients with FPG >= 7.0 mmol/L and = 7.0% and <= 12.0%.
Exclusion criteria
Exclusion criteria: 1. Patients who are unable to wear the smart ring or CGM; 2. Patients who are non-ambulatory or have severe cognitive impairment/dementia; 3. Patients who are unable to use a smartphone or do not have access to Wi-Fi; 4. Patients who are unable to answer subjective questionnaires; 5. Patients who do not accept the informed consent form; 6. Patients involved in medical disputes.
