Inclusion criteria
Inclusion criteria: 1. Age >= 60 years; 2. ASA classification I-III; 3. Patients scheduled for elective total knee or total hip arthroplasty.
Exclusion criteria
Exclusion criteria: 1. Patients who have been diagnosed with dementia or other cognitive impairments before surgery, and those with a Montreal Cognitive Assessment (MoCA) score of less than 10; 2. Patients with a history of neurological or psychiatric diseases before surgery (especially epilepsy, multiple sclerosis, Parkinson's disease, and major neurological diseases such as stroke within 3 months before surgery); 3. Contraindications for tPBM (cranial trauma or scalp injury; intracranial metal foreign bodies, such as cochlear implants, implanted pulse generators, aneurysm clips, stents, etc.; intracranial hypertension; intracranial infection; severe cardiovascular diseases, especially patients with cardiac pacemakers or cardiac stents); 4. BMI >= 40 kg/m^2; 5. Refusal to sign the informed consent form.
