Inclusion criteria

Inclusion criteria: 1. Age 60 years or older; 2. Palliative Performance Scale (PPS) assessment indicating an expected survival of no more than 6 months; 3. Demonstrated need for palliative care services, with informed consent obtained from the patient or their family member.
