Inclusion criteria

Inclusion criteria: This study aims to recruit community-dwelling older adults aged =60 years. Participants meeting all the following criteria will be eligible for inclusion: 1. Age >=60 years, regardless of gender; 2. Moderate-to-high fall risk level; 3. Resided in the community for at least 12 months within the past year and no plans to relocate within the next year; 4. Possess basic cognitive and communication abilities to complete questionnaires and physical assessments independently or with assistance; 5. Be physically capable of participating in light to moderate-intensity exercise interventions with no recent history of severe acute illness; 6. Volunteer for the study, understand its content, and sign an informed consent form; 7. Be proficient in using the WeChat app on a smartphone or have family assistance to complete follow-ups and remote exercise guidance.

Exclusion criteria

Exclusion criteria: Participants meeting any of the following criteria will be excluded from the study: 1. A confirmed diagnosis of moderate or severe cognitive impairment; 2. Presence of severe cardiovascular or cerebrovascular disease, uncontrolled hypertension, recent acute myocardial infarction, or other conditions limiting physical activity; 3. Prolonged bed rest or severe physical disability preventing completion of simple walking, balance, or strength training exercises; 4. Concurrent malignant tumors, end-stage chronic diseases, or other severe health issues; 5. Participation in any regular exercise intervention program within the past year; 6. Inability to comply with study procedures or risk of significant life changes during the study period (e.g., moving out of the community); 7. Refusal to sign the informed consent form or withdrawal of consent during the study.
