Inclusion criteria

Inclusion criteria: 1.Patients who have been diagnosed with chronic heart failure according to the diagnostic criteria for CHF in the "Chinese Guidelines for the Diagnosis and Treatment of Heart Failure" formulated by the Cardiovascular Disease Branch of the Chinese Medical Association in 2018. 2.Patients with heart failure who are classified as Class II to Class IV according to the cardiac function classification of the New York Heart Association (NYHA) in the United States. 3.Patients aged between 18 and 80 years old. 4.Patients who have received education at or above the primary school level and are able to use smart phones.

Exclusion criteria

Exclusion criteria: 1.Patients with end-stage chronic heart failure complicated by cachexia or patients with cardiogenic shock; 2.Patients with severe systemic diseases, such as severe infections, systemic lupus erythematosus, malignant tumors, etc.; 3.Patients with cognitive impairment, those who refuse to participate in this project, or those who are unable to participate in this project.
