Inclusion criteria
Inclusion criteria: 1. Age >= 18 years; 2. Meets the '8 2' criteria in the Stroke High-Risk Population Screening and Intervention Work Plan of the Brain Prevention Committee; 3. The patient or primary caregiver has a WeChat account and can operate it proficiently; 4. The patient or primary caregiver has sufficient Chinese language proficiency; 5. The subject or legal representative is able to sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Unable to operate a smartphone for trial purposes (e.g., due to vision, hearing, cognitive, or dexterity impairments); 2. Unable to access the internet in the place of residence; 3. Life expectancy < 1 year; 4. Already participated in other interventional clinical studies that may impact outcome assessment.
