Inclusion criteria

Inclusion criteria: Age: 18 years old or older Diagnosis: Physical disabilities who will receive home care services from Yang Memorial Methodist Social Service, and are medically stable.

Exclusion criteria

Exclusion criteria: Individuals with acute or unstable medical conditions requiring hospitalization, terminal illnesses with a prognosis of less than six months, psychological disorder or severe mental illness Individuals with severe cognitive impairments, e.g. diagnosed with severe dementia, intellectual disability, or other conditions preventing meaningful participation in the study
