Inclusion criteria

Inclusion criteria: 1. They satisfy the diagnostic criteria for stroke (including cerebral infarction and cerebral haemorrhage) as outlined in the Key Points for the Diagnosis of Major Cerebrovascular Diseases in China (2019), revised by the Neurology Branch and Cerebrovascular Disease Group of the Chinese Medical Association, and are confirmed by either CT or MRI imaging; 2. They are aged over 18 years; 3. They have experienced a stroke within the last four months and are in the subacute phase of stroke at the time of hospital discharge; 4. They exhibit motor impairments and require ongoing rehabilitation; 5. They and their family members are capable of using smartphones and the WeChat application, and are able to accept remote guidance and rehabilitation management through the WeChat platform; 6. They possess sufficient cognitive and language abilities to engage in rehabilitation guidance and communication related to the study; 7. They and their family members voluntarily sign an informed consent form and are willing to comply with all assessments and interventions related to the study.

Exclusion criteria

Exclusion criteria: 1. They have severe cognitive impairment (such as advanced dementia), psychiatric disorders, or disturbances of consciousness, which prevent them from understanding or cooperating with rehabilitation exercises; 2. They have severe dysfunction of vital organs (such as the heart, lungs, liver, or kidneys) or other serious systemic diseases that compromise exercise safety; 3. They exhibit severe motor impairments (such as complete paralysis) that prevent participation in a home exercise programme; 4. They are currently enrolled in other clinical trials or undergoing rehabilitation interventions that could interfere with the outcomes of this study; 5. They are unable to use WeChat or refuse remote rehabilitation guidance; 6. They require emergency treatment during the intervention period due to severe disease progression or complications (such as recurrent acute stroke, severe infection); 7. They are lost to follow-up for any reason during the intervention.
