Inclusion criteria
Inclusion criteria: 1.Age >= 60 years; 2.Patients with chronic heart failure who met the diagnostic criteria the 2024 Chinese Guidelines for the Diagnosis and Treatment of Heart Failure; 3.New York Heart Association (NYHA) functional class II~III; 4.Met the criteria for cognitive frailty proposed by the 2013 International Consensus Group: (1) self- or informant-reported cognitive decline;(2) FRAIL frailty score >=1; (3) Montreal Cognitive Assessment (MoCA) score <26, with a total score increased by 1 if the’s education level <=12 years; (4) No clinical diagnosis of Alzheimer’s disease or other types of dementia. 5.Voluntarily participate in this study sign the informed consent; 6.The distance from the patient’s home to the hospital conducting the study is within 5 km.
Exclusion criteria
Exclusion criteria: 1.With contraindications to exercise such as congenital heart disease, acute myocardial infarction, malignant arrhythmia, and the early stage of acute syndrome; 2.Comorbid with major physical diseases such as fracture, hemiplegia, or obvious limited motor function of limbs or after heart transplantation; 3.Suffering from severe mental illness; 4.Participate in other intervention studies
