Inclusion criteria
Inclusion criteria: 1. Age 18-80 years old; 2. Diagnosed with cardiometabolic diseases (including hypertension, hyperlipidemia, coronary heart disease, obesity, metabolic syndrome, sleep apnea, etc.); 3. Be able to use smartphones and WeChat mini programs; 4. Sign the informed consent form.
Exclusion criteria
Exclusion criteria: 1. Acute infectious diseases and inflammatory diseases (such as inflammatory bowel disease, vasculitis, rheumatoid arthritis, systemic lupus erythematosus and other active rheumatic immune diseases); 2. Communication disorders or severe mental and psychological diseases; 3. Patients with chronic kidney disease receiving renal replacement therapy; 4. Pregnant/lactating women; 5. No smartphone or unable to use smartphone and WeChat mini program normally; 6. Patients who are unable to take care of themselves and cannot sign the informed consent form; 7. Those who are unwilling to cooperate.
