Control group ( group A ):Receiving routine discharge care: monthly outpatient or telephone follow-up

Patients see their own medical attention on demand, without the support of traffic light system aids.

Inclusion criteria
Inclusion criteria: 1. Meets the diagnostic criteria of the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke (2023 Edition) , and is confirmed by head CT or MRI imaging examination; 2. First-time onset; 3. Clear consciousness and normal language communication skills; 4. The patient and his family members voluntarily participated in this study and signed the informed consent form, and were able to cooperate with the whole process of the study.
Exclusion criteria
Exclusion criteria: 1. Those with hemorrhagic transformation of cerebral infarction; 2. Only those diagnosed with transient ischemic attack (TIA); 3. Those with serious underlying diseases such as severe infectious diseases, autoimmune diseases, and malignant tumors; 4. Severe heart, liver, kidney and other important organ dysfunction; 5. Those whose clinical data or auxiliary examinations are incomplete and cannot meet the evaluation requirements.
