Inclusion criteria
Inclusion criteria: 1. Seek medical treatment at a public hospital and be diagnosed with at least one chronic disease; 2. Age >= 60 years old; 3. The patient has clear consciousness, no visual impairment, can understand visual information, and execute general instructions; 4.Patients or family members can use smartphones normally.
Exclusion criteria
Exclusion criteria: 1. There are communication and cognitive barriers that prevent cooperation in completing credible investigations; 2. Patients in the acute phase of the disease or those with severe changes in heart, lung, liver, and kidney function after evaluation by professional personnel; 3. Individuals with pre-existing mental disorders; 4. Refuse to interview the recorder.
