Inclusion criteria
Inclusion criteria: 1. Age >= 60 years old; 2. Diagnosed with at least 2 chronic diseases of hypertension, diabetes, coronary heart disease and osteoporosis by a second-level hospital or above; 3. Subjects who are designated hospitals, agree to participate in clinical trials and can follow up in time.
Exclusion criteria
Exclusion criteria: 1. Those who are in an acute and critical state; 2. Those who have hearing and language impairments and cannot communicate normally; 3. Combined with severe cardiovascular and cerebrovascular diseases; 4. Those who are unable to sign informed consent due to mental illness; 5. Participate in other studies at the same time; 6. Refusal to participate in survey follow-up; 7. Patients who are unable to use artificial intelligence devices or are completely unable to take care of themselves.
