Study Group:Community-Hospital-Family Interactive Management intervention for 6 months, in addition to routine care
Control Group:routine care, including health education lectures on hemodialysis and psychological counseling.

60 Years to 80 Years

Inclusion criteria

Inclusion criteria: 1 Diagnosed with end-stage renal disease and undergoing regular hemodialysis for >= 3 months. 2 Age >= 60 years. 3 Clear consciousness, normal communication, and ability to complete the survey. 4 Score <12 on the Lubben Social Network Scale-6 (LSNS-6). 5 Informed consent and voluntary participation.

Exclusion criteria

Exclusion criteria: 1 Acute exacerbation of the disease. 2 Severe mental disorders such as dementia. 3 Severe organic diseases. 4 Hearing or visual impairments.
