Inclusion criteria
Inclusion criteria: 1.Patients of any age or sex with ASA physical status I–III. 2.Scheduled for elective left thoracic surgery requiring one-lung ventilation. 3.Candidates for the "carina-to-carina" approach: on coronal CT images the distance from the tracheal carina to the first right interlobar carina (i.e., the orifice of the right upper-lobe bronchus) is <= 24 mm
Exclusion criteria
Exclusion criteria: 1.Known or anticipated difficult airway. 2.Failure of double-lumen endobronchial intubation after three attempts. 3.Endobronchial lesion within the right main bronchus. 4.Airway stenosis. 5.BMI > 28 kg/m^2 with increased peak inspiratory pressure. 6.Preoperative pulmonary function showing moderate or worse ventilatory impairment. 7.Procedures affecting the left main bronchus: left (bilateral) lung transplantation, left pneumonectomy, left sleeve lobectomy, or any other surgery compromising the left main bronchus.
