Conditions

Diuretic resistance postoperative cardiac surgery

Interventions

Control group:Conventional treatment (cardiotonic, increasing mean arterial pressure, controlling input volume, increasing diuretic dosage)
Experimental group:After intravenous loading of recombinant human brain natriuretic peptide at 1.5-2 µg/kg, refer to the instructions to maintain a dose of 0.0075-0.01 µg/kg/min for continuous intrave

Inclusion criteria

Inclusion criteria: 1. Adult patients after cardiac surgery (age>=18 years) 2. Liquid overload 3. Diuretic resistance Definition: Adequate urine volume of 0.5~1.0mL/kg/h cannot be achieved after daily intravenous furosemide dose >=80mg or equivalent doses of other diuretics. Conversion formula for different loop diuretics: oral furosemide 80mg = intravenous furosemide 40mg = oral/intravenous torasemide 20mg = oral/intravenous bumetanide 1mg 4. Willing to participate in the trial and sign the informed consent form.

Exclusion criteria

Exclusion criteria: 1. Preoperative CKD patients undergo regular dialysis treatment; 2. Use of CRRT after surgery; 3. Allergy to drugs involved in this study; 4. Systolic blood pressure is still < 90 mmHg after correction by vasopressors.
