Inclusion criteria

Inclusion criteria: 1.Age >=18 years. 2.Diagnosis meeting either: severe community-acquired pneumonia (SCAP) or treatment-refractory community-acquired pneumonia (CAP). 3.Willingness to undergo bronchoscopy. 4. Written informed consent provided by the patient or their legally authorized representative.

Exclusion criteria

Exclusion criteria: 1.Long-term residence in nursing homes. 2. Post-tracheostomy status. 3.Initial admission for CAP but subsequent ICU transfer due to hospital-acquired pneumonia (HAP) exacerbation. 4.Contraindications to bronchoscopy. 5. Failure to complete bronchoscopy within 72 hours of admission. 6.Concurrent participation in other clinical trials.
