Inclusion criteria
Inclusion criteria: 1. The ADL score (Barthel Index) is less than 100 points, and the age is greater than 65 years old. 2. There is a primary caregiver to take care of them. The primary caregiver is in good mental state, has clear consciousness, and can communicate normally with the investigator. 3. Obtain the consent of the patient and the primary caregiver for data collection.
Exclusion criteria
Exclusion criteria: 1. Those who have difficulty communicating with the primary caregiver and cannot obtain information. 2. Patients with mental illness disorders or violent tendencies. 3. Patients who have been hospitalized multiple times. 4. Pressure injuries that occur in hospitals. 5. Inconsistent judgments on pressure injuries.
