60 Years to No maximum

Inclusion criteria

Inclusion criteria: 1. Age >=60 years old; 2. Be diagnosed with no less than two chronic diseases in a secondary-level or higher hospital; 3. Take at least one prescription drug for a duration of more than 6 months after discharge; 4. Permanent residents of Fuzhou urban area who have resided for more than six months.

Exclusion criteria

Exclusion criteria: 1. Severe cognitive impairment or mental illness, unable to understand the research requirements; 2. Combined with severe heart, liver and kidney insufficiency and malignant tumors; 3. Has participated in other clinical trials that may affect the results of this study.
