Inclusion criteria

Inclusion criteria: For chronic heart failure patients with stable condition but requiring continuous monitoring (1) Patients over 18 years old; (2) Patients who are able to use smartphones; (3) Diagnosed with heart failure.

Exclusion criteria

Exclusion criteria: (1) Patients with congenital heart disease; (2) Patients with cognitive impairments or mental illnesses; (3) Patients who are currently participating in other related studies; (4) Suffering from known severe progressive diseases (such as malignant tumors, liver dysfunction, or liver disease) or diseases that cause extreme exhaustion in patients, with an estimated survival time of less than 12 months; (5) Pregnant or prospective pregnant women; (6) Any situation that may interfere with the research process, such as dementia, paralysis, alcohol abuse, etc.
