Surgery for Hip Fracture in the Elderly

Interventions

Hydromorphone group:Simultaneously with the completion of the iliofascial block injection, the hydromorphone group (H.Group) was injected intravenously with hydromorphone 0.5 mg (3 ml) over a 1-minute
Saline group:At the same time as the iliofascial block injection was completed, 0.9% saline (3 ml) was injected intravenously in the saline group (N.Group) for a duration of 1 minute.

Inclusion criteria

Inclusion criteria: 1. ASA class II-III; 2. Age >=65 years; 3. Scheduled to undergo the following procedures under spinal anesthesia: total hip arthroplasty, femoral head replacement, intramedullary nailing, or screw fixation.

Exclusion criteria

Exclusion criteria: 1. Failure to obtain informed consent; 2. Lacks capacity to provide informed consent; 3. Allergy to ropivacaine, hydromorphone, or related components; 4. Contraindications to neuraxial anesthesia or iliac fascia block; 5. Opioid abuse or pathological pain requiring long-term analgesic therapy; 6. Gastric retention or paralytic ileus; 7. Psychiatric disorders.
