Inclusion criteria
Inclusion criteria: 1. Age 65-80 years; 2. Meet the diagnostic requirements for cerebral infarction as outlined in the Chinese Guidelines for the Diagnosis and Treatment of Acute Ischemic Stroke 2023, with confirmation by CT or MRI; 3. Presence of nutritional risk within 2 weeks after onset (MNA-SF score <=11); 4. Stable vital signs and condition in a stable phase; 5. Clear consciousness; 6. Ability to use a smartphone independently or with assistance from family members; 7. Informed consent and voluntary participation in the study.
Exclusion criteria
Exclusion criteria: 1. Comorbidities including severe organ failure (e.g., heart, liver, kidney), malignant tumors, or hematologic diseases; 2. Language or communication barriers preventing proper understanding or cooperation; 3. Severe psychiatric disorders; 4. Death or transfer to another hospital/department during the study.
