65 Years to 90 Years

Inclusion criteria

Inclusion criteria: Consistent with diagnosis of hip fracture by x ray or CT; 1. age >= 65;2.PFNA internal fixation with surgical indications; Without cognitive impairment;3. able to read text and use WeChat; 4.The patient and family members agree to participate in this study; 5. Residents who live in nursing institutions or communities in Gulou District, Jin' an District, Cangshan District, and Taijiang District after discharge; 6. Choose home or community rehabilitation.

Exclusion criteria

Exclusion criteria: 1.patients who are not undergoing hip fracture surgery for the first time ; 2. associated with systemic important viscera function failure or severe disease of heart, brain, nervous system, etc.; 3.compliance claim; 4. disorder, communication disorders.
