Inclusion criteria
Inclusion criteria: 1. Diagnosis of CHF in accordance with the "2018 Chinese Guidelines for the Diagnosis and Treatment of Heart Failure" ; 2.New York Heart Association (NYHA) functional class II to IV; 3.Age >= 18 years; 4.Education level: primary school or above; 5. Able to master the operation method of smartphone applications. 6. Voluntary participation and completion of informed consent form.
Exclusion criteria
Exclusion criteria: 1. Post heart transplantation; 2.Recent encounter of major life events; 3.Malignant tumor; 4.History of psychological disorders or psychiatric diseases.5.In-hospital death; 6.Interruption of follow-up outside the hospital for various reasons.
