Inclusion criteria
Inclusion criteria: (1) Diagnosed with type 2 diabetes mellitus (T2DM) according to the diagnostic standards established by the Chinese Medical Association; (2) Aged >=60 years with established health records in community health service centers; (3) On medication for >=3 months; (4) Cognitively intact with adequate communication skills to complete all assessments; (5) Willing to provide informed consent and voluntarily participate in the study.
Exclusion criteria
Exclusion criteria: (1)Individuals with severe somatic or mental disorders; (2)Those with hearing, speaking, reading, or writing impairments; (3)Patients with acute or severe diabetic complications (e.g., ketoacidosis, severe diabetic foot infections, end-stage diabetes); (4)Individuals with cognitive impairment or diagnosed with Parkinson's syndrome, dementia, or disability;(5)Patients who have participated in other similar studies within the past year.
