Inclusion criteria

Inclusion criteria: 1. Middle-aged and elderly residents (aged >= 45 years) with at least one common chronic disease (such as hypertension, diabetes, hyperlipidemia, and coronary heart disease diagnosed by medical institutions); 2. Able to find a group of middle-aged and elderly residents with chronic diseases who have a pre-existing social relationship (such as relatives or friends) to participate in the team; 3. Possess an Android smart phone and be proficient in using application software (such as taking photos); 4. Have good cognitive function and communication skills; 5. Have good motor function and be able to complete regular exercise.

Exclusion criteria

Exclusion criteria: 1. Presence of other neurological disorders that affect motor function; 2. Installation of a cardiac pacemaker; 3. History of epilepsy, as well as concurrent severe primary diseases such as heart, liver, kidney, metabolic system disorders that are not suitable for exercise; 4. Cognitive communication disorders that make it difficult to understand or execute exercise intervention; 5. Participation in other clinical studies.
