Inclusion criteria
Inclusion criteria: 1. Meeting the Tokyo Guidelines 2018 (TG18) diagnostic criteria for acute cholangitis, requiring at least one item from each category (A + B + C): A: Fever or chills; body temperature>38°C or WBC count 10×10?/L B: Jaundice ; liver dysfunction (total bilirubin >34 µmol/L OR AST/ALT/ALP/GGT >1.5×ULN) C: Imaging findings: Biliary dilation or confirmed etiology (stones, tumor, stricture) on imaging 2. Age >18 years 3. Physically eligible for surgery 4. Written informed consent obtained from patients and families
Exclusion criteria
Exclusion criteria: 1. Uncorrectable severe coagulopathy ; 2. Absolute contraindications to PTCS/ERCP ; 3. Severe organ dysfunction precluding safe anesthesia/invasive procedures; 4. Patients with septic shock refractory to preoperative antimicrobial therapy and hemodynamic support
