Inclusion criteria

Inclusion criteria: 1.Based on the diagnostic criteria outlined in the National Primary Care Diabetes Management Handbook (2019) jointly issued by the Diabetes Society of the Chinese Medical Association and the National Primary Diabetes Prevention and Control Office, patients aged >=40 years and =11.1 mmol/L at any time of the day. (2)Fasting plasma glucose (FPG) >=7.0 mmol/L. (3)Blood glucose >=11.1 mmol/L 2 hours after a glucose load. 2. HbA1c >6.5%, indicating suboptimal glycemic control. 3. All participants voluntarily agreed to take part in the intervention study. Those in the intervention group, with or without family assistance, used smart devices for daily management and signed informed consent forms.

Exclusion criteria

Exclusion criteria: 1. Patients with severe complications, such as diabetic ketoacidosis or hyperosmolar hyperglycemic nonketotic coma. 2. Patients with severe comorbid conditions, including malignancies, acute cardiovascular or cerebrovascular diseases, acute or chronic pancreatitis, or pancreatic insufficiency. This also includes patients with severe cardiac diseases (e.g., NYHA class II or above heart failure, left bundle branch block, or third-degree atrioventricular block) who cannot tolerate moderate-intensity exercise, as well as patients with uncontrolled severe hypertension (systolic blood pressure >160 mmHg) or other conditions deemed unsuitable for inclusion based on comprehensive geriatric assessment. 3. Other exclusion criteria, such as inability to answer questions clearly, lack of familiarity with WeChat or the internet without caregiver assistance, or current participation in other clinical studies. 4. Refusal to sign the informed consent form.
