Inclusion criteria

Inclusion criteria: Age >= 65 years ASA Physical Status Classification II or III Surgical duration between 1–4 hours History of diabetes mellitus =3 years, with preoperative fasting blood glucose controlled =10 mmol/L Preoperative Mini-Mental State Examination (MMSE) score >27 Scheduled for laparoscopic surgery, with no surgical contraindications

Exclusion criteria

Exclusion criteria: Patients with elevated intracranial pressure (e.g., hydrocephalus, brain tumors, intracranial space-occupying lesions). Patients with severe vision or hearing impairment preventing effective communication. Preoperative moderate-to-severe anemia (hemoglobin <90 g/L). Patients undergoing emergency surgery or procedures converted to open surgery intraoperatively. Patients with preexisting psychiatric disorders. Patients with severe arrhythmias, cardiac/hepatic/renal failure, or other major organ dysfunction. Preoperative arterial oxygen partial pressure (PaO2) <60 mmHg.
