Experimental group:Intervention content: After enrollment, patients will receive regular home hospice care services, which are as follows:
Develop a personalized care plan: After the patient is enroll
(3) Medication guidance: medication guidance to patients and caregivers and how to deal with side effects of medications
(4) Daily care and guidance: Train caregivers in symptom recognition, daily care skills, safe transfer techniques and advance care planning to improve caregivers' ability to care at home
(5) Psychosocial support: Provide psychological support, emotional counseling, and related resources to help patients cope with the impact of the disease and emotional distress.

Inclusion criteria
Inclusion criteria: (1) Patients diagnosed with malignant tumors by histology or cytology and diagnosed with stage III or IV cancer according to the TNM staging system; (2) Age >= 18 years old; (3) Able to understand Chinese characters and express subjective wishes; (4) Willing to cooperate with this study after explaining the research purpose; (5) Know the condition.
Exclusion criteria
Exclusion criteria: (1) Combined with serious mental illness or consciousness disorder; (2) Those with severe cognitive impairment; (3) Language expression disorders.
