Inclusion criteria
Inclusion criteria: 1.Suffering from dementia, with cognitive assessment results indicating mild or moderate dementia; 2.Having at least one cardiovascular disease (including hypertension); 3.Having suffered from cardiovascular disease for no less than 6 months; 4.Being able to speak and understand Chinese; 5.Being able to express the ability to voluntarily participate in the study
Exclusion criteria
Exclusion criteria: 1. The life expectancy of the patient is less than 12 months; 2. Living in a nursing home or long-term care facility; 3. Family caregivers with whom no contact is maintained; 4. Participating in other research involving chronic disease management
