Inclusion criteria

Inclusion criteria: 1) Age >=18 years old, gender unlimited. 2) A clinician diagnosed with chronic kidney disease (CKD) stage 1-5. 3) Have a smart phone and can skillfully use wechat app, willing to accept intervention through this platform. 4) Able to understand the procedures and methods of this study, willing to sign informed consent and strictly follow the clinical study protocol to complete this study. 5) Patients undergoing follow-up visits in our hospital.

Exclusion criteria

Exclusion criteria: 1) Patients with CKD who are receiving alternative treatments (such as hemodialysis, peritoneal dialysis, or kidney transplantation). 2) There are serious cardiovascular, cerebrovascular, kidney or other vital organ dysfunction diseases. 3) Those who are participating in other similar research projects. 4) Unwilling to cooperate, and other reasons are not suitable for participants in the experiment.
