Inclusion criteria
Inclusion criteria: 1. Age 60-79 years old; 2. Discharged patients who meet the diagnostic criteria for chronic heart failure in the Chinese Guidelines for the Diagnosis and Treatment of Heart Failure 2018; 3. New York Heart Disease Academy (NYHA) cardiac function grade II~IV; 4. No mental disorders and cognitive impairments, able to communicate normally; 5. Able to use a smartphone independently or with the assistance of a caregiver; 6. Informed consent and voluntary participation in this study.
Exclusion criteria
Exclusion criteria: 1. Refractory end-stage heart failure; 2. Coexisting serious diseases or complications, such as severe valvular disease, atrial fibrillation, malignant tumors, serious infections, etc.; 3. Those who are unable to monitor the capacity status by themselves.
