Inclusion criteria
Inclusion criteria: (1) Age >= 18 years old, meeting the diagnostic criteria for cerebral infarction and cerebral hemorrhage in the Diagnostic Points of Various Major Cerebrovascular Diseases in China 2019 issued by the Cerebrovascular Disease Group of the Neurology Branch of the Chinese Medical Association, diagnosed with first onset of illness confirmed by CT or MRI. (2) Those who are in stable condition after medical treatment, have clear consciousness, stable vital signs, and can communicate in speech or writing, and have the ability to read and write independently. (3) National Institutes of Health Stroke Scale (NIHSS) score <= 15, Rankin Revised Scale (MRS) <= 3. (3) Permanent residents of Chengdu (=6 months). (4) Informed and voluntary participants in the program (signed consent form by the patient or immediate family members).
Exclusion criteria
Exclusion criteria: (1) Those who have participated in other self-management training programs and are participating in other research programs. (2) Those who have a combination of other serious diseases (patients with malignant tumors, severe hepatic or renal insufficiency, severe trauma, respiratory failure, severe cardiac arrhythmia, frequent angina pectoris, heart failure, myocardial infarction, etc.), and those who are blind, deaf, or mute. (3) Persons with impaired consciousness or cognitive impairment, with a score of <8 on the Abbreviated Mental Test Scale (AMT), or persons with psychiatric disorders, or persons suffering from sensory aphasia who are unable to communicate.
