Inclusion criteria
Inclusion criteria: (1) Meets the diagnostic criteria for chronic diseases and has one or more chronic diseases (such as coronary heart disease, stroke, hypertension, diabetes, etc.). (2) Age >= 60 years old. (3) Community residents (with household registration in the city, or having lived here for 6 months or more). (4) Voluntarily participates in this study and signs an informed consent form.
Exclusion criteria
Exclusion criteria: (1) Difficult to communicate with. (2) Vulnerable groups other than the elderly/illiterates, including those with mental illness, cognitive impairment, critically ill patients, etc.
