Inclusion criteria

Inclusion criteria: (1) Elderly 1 Age 60 years or older. 2 Meet the diagnostic criteria for chronic diseases, with one or more chronic diseases (such as coronary heart disease, stroke, hypertension, diabetes, etc.). 3 Community residents (with household registration in the city, or having lived here for 6 months or more). 4 Able to independently use a smartphone. 5 Voluntarily participate in this study and sign an informed consent form. If the participant is unable to read and sign the informed consent form due to lack of legal capacity or other reasons, the guardian must act on their behalf in the informed consent process and sign the informed consent form. (2) Family members of the elderly 1 Age 18 years or older. 2 Able to independently use a smartphone. 3 Voluntarily participate in this study and sign an informed consent form.

Exclusion criteria

Exclusion criteria: (1) Elderly 1 Individuals with severe chronic diseases and complications, such as patients with severe complications from diseases like hypertension and diabetes (excluding patients in the early stages of diseases such as diabetes and hypertension, with mild symptoms and well-controlled conditions). 2 Vulnerable groups other than the elderly, including those with mental illnesses, cognitive impairments, critically ill patients, pregnant women, illiterates, and others. (2) Family Members of the Elderly Vulnerable groups other than the elderly, including those with mental illnesses, cognitive impairments, critically ill patients, pregnant women, illiterates, and others.
