Inclusion criteria

Inclusion criteria: 1. Diagnosed with chronic diseases by a secondary or higher-level hospital, with the number of chronic disease types = 2. Chronic disease types are defined as chronic diseases according to the 11th edition of the International Classification of Diseases, self-reported and confirmed by a hospital, including hypertension, diabetes, hyperlipidemia, coronary atherosclerotic heart disease, chronic obstructive pulmonary disease, etc. 2. Age >= 40 years old; 3. Conscious, without intellectual disability, and having complete cognitive and behavioral abilities. 4. Informed consent and voluntary participation in this study.

Exclusion criteria

Exclusion criteria: 1. Those who are unable to cooperate or have difficulty in communication due to deafness, dumb, dementia, etc; 2. Those with critical or severe diseases and unable to cooperate in completing the study.
