Intervention group:72 patients with maintenance hemodialysis were randomly divided into control group and intervention group, 36 in each group. The control group received routine home diet self-manage
Control group:Receive regular home diet self-management education

Inclusion criteria

Inclusion criteria: Inclusion criteria: 1) Patients met the diagnostic criteria established in the 2012 U.S. Kidney Disease Foundation (KDIGO) guidelines; 2. Chronic kidney disease stage 5 patients, the condition is relatively stable; 3.Age >=18 years old; 4. Regular MHD treatment >=3 months; (5) Patients or family members can correctly use wechat mini programs, public accounts and other networks.

Exclusion criteria

Exclusion criteria: Exclusion criteria: patients with cognitive impairment, consciousness impairment, speech comprehension dysfunction, mental illness.
