Peri-dialysis chronic kidney disease

Interventions

Test group:Experimental content: Distribute test papers to patients, identify specific problems they have, and clarify change goals
Implementation time: The first day after the patients admission
Intervention frequency: one lecture per day during hospitalization (7-10 times)
Implementation methods: expert lectures, small lectures, interactive learning, group discussions, case sharing, etc
Result assessment: Distribute test papers again to test the completion status of patients' goals. Based on the preliminary experimental results and patient feedback, the research team adjusted the app
control group:The control group will receive routine nursing care and self-management education.

Inclusion criteria

Inclusion criteria: 1.EGFR =18 years old; 3.Self management score <=74 points; 4.Has a certain level of reading ability, or lacks reading ability but family members can assist and cooperate; 5.Voluntarily participate in this trial and accept subsequent follow-up.

Exclusion criteria

Exclusion criteria: 1.Combining cardiovascular and cerebrovascular complications such as stroke, malignant hypertension, and acute myocardial infarction; 2.Patients with acute renal impairment; 3.Patients with combined cognitive impairment, aphasia, hearing impairment, and mental illness; 4.Patients with infectious diseases such as hepatitis, gastrointestinal bleeding, pneumonia, and malignant tumors.
