Inclusion criteria
Inclusion criteria: Age >=18 years old; Patients with previously diagnosed hypertension, currently taking antihypertensive drugs, but blood pressure is still uncontrolled; Mean baseline blood pressure measured on the same day and the next day (three times, using the second and third blood pressure measurements, 1-2min between each measurement) : systolic blood pressure >140 mmHg and/or diastolic blood pressure >90 mmHg; For digital interventions, participants need to be willing to conduct self-monitoring and be able to use wechat mini programs or ones that family members can assist in using.
Exclusion criteria
Exclusion criteria: NYHA IV patients with heart failure; Acute stroke occurred in the past month; Patients with advanced tumors; Pregnant women and lactating women; After guidance and training, wechat mini program can not be used.
