Inclusion criteria

Inclusion criteria: Doctor inclusion criteria: 1. Worked in community service center for more than 1 year 2. Qualified as a medical practitioner 3. Skilled use of smart devices 4. Agree to participate in the study and sign the informed consent Patient inclusion criteria: 1. Age >=18 years old 2. Visit at the community service center 3. Ability to understand and follow research procedures 4. Agree to participate in the study and sign the informed consent

Exclusion criteria

Exclusion criteria: Doctors exclusion criteria: 1. Plan to transfer or take leave for more than 1 month during the trial period 2. Participate in other trials that may affect the results of this study Patient exclusion criteria: 1. People with severe mental illness 2. Patients who could not complete the questionnaire independently 3. Patients who are not expected to complete follow-up within 3 months
