Inclusion criteria

Inclusion criteria: Patients aged >=60 years old in the Department of Nephrology and General Medicine of our hospital; Meet the KDIGO diagnostic criteria for chronic kidney disease, Abnormal kidney structure or function that lasts for more than 3 months is chronic kidney disease. Subjects who agree to participate in the program and sign the informed consent form.

Exclusion criteria

Exclusion criteria: Life expectancy is less than half a year; Severe hearing impairment that prevents communication with the assessor.
