Experimental group :The experimental group will use the smart health management platform for follow-up intervention management.
Control group:The control group will receive conventional intervention management.

Inclusion criteria

Inclusion criteria: 1. Aged between 65 and 85 years old; 2. Meeting the frailty diagnostic criteria of the Fried scale; 3. Agree to participate in this clinical trial and sign the informed consent form.

Exclusion criteria

Exclusion criteria: Currently receiving or previously participated in interventions or clinical trials related to frailty.
