Inclusion criteria
Inclusion criteria: Qualitative research: 1. Age 65-80 years old; 2. Diagnosed as coronary atherosclerosis; 3. The score of Fried weak phenotype is >= 1; 4. The discharge time is >=6 months; 5. Informed consent and voluntary participation in this study. Quantitative research: 1. Age 65-80 years old; 2. Diagnosed as coronary atherosclerosis; 3. The score of Fried weak phenotype is >=1; 4. Elected discharged patients; 5. Having caregivers and being able to use smart phones conveniently; 6. Informed consent and voluntary participation in this study.
Exclusion criteria
Exclusion criteria: Qualitative research: 1. The diagnosis includes hepatic and renal insufficiency, malignant tumor, thyroid disease, peptic ulcer, immune system disease and hyperuricemia; 2. Unable to communicate normally. Quantitative research: 1. The diagnosis includes hepatic and renal insufficiency, malignant tumor, thyroid disease, peptic ulcer, immune system disease and hyperuricemia; 2. Patients who did not undergo surgery during this hospitalization; 3. Poor mental state prevents normal communication; 4. The condition suddenly turned critical and the discharge was suspended.
